REGISTRATION REQUIREMENTS (Click for *.pdf version)

A parent must accompany the student and be prepared to provide evidence of compliance with each of the following requirements:

Legal Proof of AgeBirth Certificate; Baptismal Certificate; Passport; Statement by the County Registrar or Recorder; or Notarized Affidavit from the parent/guardian. HOSPITAL CERTIFICATES OF BIRTH ARE NOT ACCEPTABLE.
Proof of Residency Current Gas, Electric, or Water Bill, &/or Notarized Landlord Verification.
Polio Immunizations Dates for 3 immunizations. Booster needed if 3rd dose was before fourth birthday.
DTP (Diphtheria, Tetanus, Whooping Cough) before fourth birthday. Dates for 4 immunizations. Booster needed if 4th dose was and Pertussis -
Measles Immunization Date of 2 immunizations (both after the first birthday).
Rubella Immunization Date of 1 immunization.*
Mumps Immunization Date of 1 immunization.*
TB (Mantoux) Skin Test Within one year of entering kindergarten. Must present signed evidence of test results.
Hepatitis B Dates for 3 immunizations.
Varicella Vaccine Date of 1 immunization (or verification of disease signed by doctor).
*Booster needed if immunization was before first birthday

Exemption from immunization is granted for medical reasons (Physician’s Written Statement), or for personal belief (Parent’s Written Statement).
TUBERCULOSIS SKIN TEST

Children are required to have a skin test for tuberculosis before enrolling in kindergarten or from outside the state of California. If the child has a significant (positive) skin test reaction, this means he/she has been exposed to tuberculosis and will need an x-ray and be evaluated for possible treatment. It does not necessarily mean that the child has the disease – very few do. Children are not excluded from school because of a positive skin test or because they are receiving medication for tuberculosis.

CHILDREN ARE NOT ENROLLED IN SCHOOL UNTIL THE TUBERCULOSIS SKINTEST (MANTOUX) HAS BEEN ADMINISTERED.
